What is the Louisiana Adoption Registry? The Louisiana Adoption Registry is a registry established by the Department of Social Services to facilitate voluntary contact between adult adoptees, their birth parents and/or siblings. Who may register? You may register if you are over 18 and your adoption, your child’s or your sibling’s adoption was legally finalized in the State of Louisiana. How do I get on the registry?Contact the registry at (225) 342-9922 or 1-800-259-2456 and request a Voluntary Registry packet which will provide all the information necessary for registration. The process includes completion of an affidavit and a $25.00 fee. There is also a counseling requirement that must be met prior to voluntary release of any identifying information, but this is not required in order to register. Where do I write?Should you prefer to write us, send your letter to: The Louisiana Voluntary And indicate whether you are an adoptee, birth parent or sibling and include your name, mailing address, and phone number. We will send you a registry packet or call you if we have questions. If you have further questions, you may call the Registry at (225)-342-9922 or 1-800-259-2456.
