Birthparent Sample Letter Requesting Non-Identifying Information, (NON-ID) Department of Social Services Adoption Program Subject: Request for non-identifying information Reference: On ______________I gave birth to a son/daughter at (name of hospital) in (name of city) and placed he/she for adoption through (name of agency or private attorney). The purpose of this letter is to clarify the details of this adoption and to request non-identifying information on the placement of my son/daughter. Please correct or provide the date of birth, sex of the child, maternity home and hospital if necessary, incorrect or unknown. Also, please send the non-identifying information on the adopting parents. I am interested in their ages, states of birth, states of residence at the time of placement, religion, education and occupation. Did they have or adopt other children, and if so, what are their ages and genders? How long were the parents married before adopting? (If you have any medical problems mention them here).
